Pharmacies. The Lombardy Region’s instructions for joining the new telemedicine services

From June 10th, pharmacies will be able to join on the Portal https://portale.federfarmalombardia.it, selecting which service they want to provide. The selected services will be visible to citizens via the Open Pharmacy app. The services that can be provided from June 18th are: blood pressure holter, cardiac holter and ECG. The Patient accesses the service only upon presentation of the non-NHS white prescription prescribed by mmg or pls.

06 JUN – As part of the resolution approved last May for the testing of new pharmacy services, the Lombardy Region recalls that

the first service to start will be the one relating to Telemedicine. From June 10th, pharmacies will be able to join on the Portal https://portale.federfarmalombardia.it, selecting which service they want to provide. The selected services will be visible to citizens via the Open Pharmacy app.

The services that can be provided from June 18th and the related remunerations are as follows:
– Holter pressure: €. 42.20;
– Cardiac Holter: €. 63.30;
– Ecg: €. 25.00.

Mandatory training
The pharmacies participating in the trial indicate – during the registration phase – the personal details of at least one pharmacist registered with the Order, responsible for activities relating to the provision of telemedicine services, certifying that mandatory training has been completed.
Mandatory training includes participation in the course relating to the activity provided, which can be accessed at https://www.fadfofi.com/index/corsi-farmacia-dei-servizi.php.

Accredited facilities

The pharmacies participating in the trial choose – during the registration phase – a provider who uses “Tele-reporting carried out by Accredited Health Facilities (registered with ASAN)” or accredited in other Italian regions as indicated to the regions by the Ministry of Health and which is reported below:
– “reporting with remote connection must necessarily be carried out by doctors operating in specialist centers of structures accredited with the NHS, both in the region of delivery and in other regions”.

The certifications, which are collected by Federfarma Lombardia, are shared with the ATS which can verify “compliance with the accreditation requirement required for the Healthcare Facilities that deal with tele-reporting and that operate in collaboration with the Pharmacies”.

Suitable and equipped spaces
Each pharmacy organizes suitable and equipped spaces to provide the expected telemedicine services, in compliance with Annex 1 DGR 848/2023.

Reference target and access methods
In the first application, this service is aimed at patients in Lombardy (registered with the NAR) suffering from cardiovascular diseases (we reserve the right to extend the Target during the course of the trial).
The Patient accesses the service only upon presentation of the non-NHS white prescription prescribed by the GP or PLS.

It can carry out up to a maximum of 3 services per type, i.e. a total of 9 investigations.

The medical prescription (on a white prescription) must be:
– duly signed by GP or PLS;
– with indication of the Telemedicine Service to which the patient must undergo
– with indication either that the patient is affected by Cardiovascular pathologies or the exemption code associated with the Cardiovascular pathology.

In the pharmacy
The patient then goes to one of the registered pharmacies with the prescription as described above and books the service at the pharmacy.

At the time of service the pharmacist collects:
1. the citizen’s self-declaration of being in possession of an exemption associated with cardiovascular disease;
2. the self-declaration of not having used 3 other telemedicine services in the Service Pharmacy project (maximum 3 ECG, maximum 3 Pressure Holter, maximum 3 Cardiac Holter)
3. the privacy consent uses the model made available by the Provider chosen by the pharmacy.

The pharmacist then:
– connects to the provider’s software to record personal information,
– install the devices to carry out the requested service;
– informs the Patient about the availability times of the report;
– print and deliver, in total respect of privacy, the report to the Patient, inviting him to show it to his GP during the next visit or in a short time based on the findings;
– receives the signature of the Patient on the delivery receipt of the report;
– archives the self-declaration, privacy consent and prescription for 5 years.

To report the services performed, pharmacies will use the platform https://www.dottorfarma.it.

ARIA, as already happens for other services, will collect the reported services, which will be included in the Invoice for SISS Services.

The services carried out will be billable with the SISS invoice for September 2024.
