TAVULLIA – Patient zero is from Tavullia and is leading the way at the Pesaro Urbino Ast for the telemedicine service and care of nephrology patients through video dialysis. The project, managed by the Nephrology and Dialysis department of the Fano hospital, allows the patient to communicate in real time, via audio and video, with the healthcare staff for advice, support and operational instructions for the management of the dialysis treatment directly from their home, without the need to go to hospital.

«Patient zero is from Tavullia – explains the director of the Complex Nephrology and Dialysis Operating Unit Marina Di Luca – and the service will be extended to patients who have clinical, logistical and organizational needs. It is a cutting-edge video dialysis tool to support patients with chronic kidney disease on home peritoneal dialysis therapy. A very important proximity medicine device: the specialist arrives directly at the patient’s home and through technology visits him and does a teleconsultation, thus avoiding the patient having to go to hospital. Lower costs for the national health service, but also the guarantee of adequate care and continuous monitoring of the user’s health. The initial phase proceeds very well with the full satisfaction of the patient and the healthcare staff. Thanks to all the healthcare, technical and administrative staff who made it possible to achieve this result.”

The data relating to vital parameters are transmitted, in real time, from the patient’s home and the hospital facility, directly to the operations center of the Nephrology and Dialysis department of the Fano Hospital, through a mobile device (totem) installed at the person’s home on dialysis (in case of home therapy). Telemedicine allows doctors to adopt the most suitable therapeutic choices, to intervene and resolve any problems early, thus reducing urgent visits to the emergency room and unplanned hospitalizations.

Life quality

The regional health department highlights that telemedicine and video dialysis make it possible to improve the quality of life of patients, taking advantage of the best technologies and home services, so as to also reach patients who live in the most internal areas of the region and those who have difficulty reaching the hospital. An approach that allows the optimization of resources and personnel of the Regional Health System, to give concreteness to widespread healthcare across the territory and capable of providing responses to the growing chronic conditions linked to the progressive aging of the population.

«The patient is followed by the team of the Peritoneal Dialysis clinic in Fano which currently has around 30 patients under treatment and over 250 in the pre-dialytic phase of the disease – adds Silvio Di Stante, Head of Home Dialysis (Peritoneal and Hemodialysis) and MaReA Clinic (Advanced Kidney Disease) – video dialysis will make it possible to shorten the distances between the hospital center and patients with important potential developments in remote multidisciplinary care in the area. More timely support interventions for patients and caregivers will be possible, fewer ordinary and emergency visits to hospital, greater human closeness to patients, obtaining better quality of life and assistance.”
