Electronic Health Record, unique access to the patient’s medical history
The Electronic Health Record (FSE) is a fundamental tool for improving the quality and effectiveness of care in the field of sanitary system. A tool for the citizen of Campania that contains digital health and social-health data and documents generated following healthcare services received. With the Electronic Health Record you can, therefore, consult your medical history and, at the same time, allow it to be consulted, safely and with respect for privacy, by doctors and healthcare professionals during the treatment process. In Campaniayou can access your own ESF with a few clicks by connecting to Citizen’s Health Portal (https://sinfonia.regione.campania.it) or from the App Campania in Health downloadable from the Stores.
Give consent to consult the ESF to make it visible to your doctor
To make the information in the website accessible and consultable Health Record to the healthcare professionals who will take care of him (the family doctor, a specialist, in case of access to the emergency room…), it is necessary for the patient to give consent either through Sinfonia Portal or theApp by accessing the specific section “Consent Management”.

It is possible, however, to object to the uploading of data and documents relating to healthcare received before 19 May 2020 by next 30 June.
