They have been in operation since today, Monday May 27th, the three new district territorial operations centers (COT) of Cesena, Cesenatico and Savignano sul Rubicone, serving the territory of the Cesena Valle Savio Union and the Rubicone Mare Union.

The organizational model (11 those planned by the Romagna Local Health Authority, normally one per District with the exception of some districts which in relation to the oro-geographical context will have a higher number) finds its roots within the Ministerial Decree 77 and is confirmed in the recent DRG 796 of our Region – rules that illustrate the strengthening of territorial assistance – representing the key to internal integration between the service chain and the professionals involved in the different places and levels of care, ensuring continuity, accessibility and complementarity of health and social care.

Cot guarantees a complete vision of the citizen’s state of health, so as to allow the individual professionals involved in the different degrees of care to interact and decide on the most appropriate interventions: this allows timely decisions while respecting the appropriateness of the resources used, as well as increasing sharing and integration with general practitioners, paediatricians of free choice and in general with all the nodes of the territorial and hospital network.

There Cot of the Cesena District it is located at the territorial structure of Corso Cavour 180, on the first floor of the recently renovated building.

In the Rubicone District there are two Cots: the District Headquarters of Cesenatico is located at ground floor of the Costa Community House in via Abba 102the District Headquarters of Savignano is located at the Rubicone Community House in Corso Perticari 119.

All the structures required extraordinary maintenance interventions carried out by the Technical Activities Office (Cesena cost 253,700 euros, Cesenatico cost 152,500 euros, Savignano cost 152,500 euros, financed by PNRR Mission 6 Health funds).

The Cots are operational from Monday to Friday, from 8am to 3pm (the Cesena Cot will also guarantee opening on Saturday mornings from 8am to 2pm) and the service is carried out by professionals adequately trained in telephone answering with the function of triage, as well as multidimensional team assessments and intervention planning.

“The activation of the district COTs of Cesena, Cesenatico and Savignano sul Rubicone on schedule is further confirmation of the great investment of the Romagna Local Health Authority in strengthening local services – they underline Paola Ceccarelli, Director of the Cesena Valle Savio District and Francesco Sintoni, Director of the Rubicone District – with new forms of coordination between professionals in taking care of people and in connecting services in different care settings. Just like the Cau and the Community Houses, the COT are part of the development of the network of local services to ensure that the first treatment and assistance option is the one in proximity to the home. In essence it is an “intelligent power plant”, driving the resources of a territory, not only of the various public sector entities, to move in the most coordinated way possible”.

In other words, it is a real “direction”, which takes care of the patient’s different needs, organizing the most appropriate responses. The health worker or social worker who first comes into contact with the citizen and identifies a new health need (even temporary) activates the Cot which puts the resources online to identify the best solution. Furthermore, the Cot will become one of the main interfaces with the 116117 Service currently in the operational design phase.

Who can contact the Territorial operations center? Not the private citizen, but one of the subjects who are taking care of his assistance, that is, the doctor or pediatrician, a hospital or other nodes of the social and healthcare network. In fact, it can be activated by all professionals in the area and in the hospital, both in the healthcare and social sectors: general practitioners, paediatricians, social workers, hospital, emergency room, continuity of care doctors, community nurses, specialists , other healthcare professionals in the network.

THE recipients of its activity they are the so-called “fragile” citizens, predominantly elderly people, or those with disabilities or disabling illnesses, who in most cases present a picture made up of multiple pathologies and therefore require 360-degree care of them. The objective is also to keep the person within their own life context and relational network as much as possible.

For example, the general practitioner may need, for one of his patients, to activate a course in a community hospital or the assistance of a nurse and home health worker; or, for a citizen returning from hospital after an acute phase of his pathology, it is necessary to better organize the return home with a bed, a wheelchair and a visit to home care and the OSS: in this last case a contacting the Cot can also be the hospital department itself. However, there may be an elderly patient who, after meeting with the social service, must be placed in a residential facility because the family network is no longer able to provide for all his needs and therefore in this case an evaluation process is activated. Again, a patient unfortunately in the terminal stage of the disease, who needs a path to relief from suffering, through the activation of the home palliative care team and the support of dedicated volunteers for him and his caregivers.
