Help the patient stay at home and thus improve their living conditions. The “Choosing to stay at home” project, financed with the European Social Fund Plus (ESF+) 2021-2027, which involves the Colline dell’Albegna District Area, aims to support the development of a home care system capable of dealing with the needs of the person in the contexts of health and family care, and respond adequately to the patient’s care needs by encouraging the patient’s stay at home for as long as possible and promoting a better outcome.
“Choosing to stay at home” is a path that involves the implementation of the following stages
1. “Hospital-territory continuity of care services” which provides financing for home care and assistance for elderly people over 65 with temporary limitations of autonomy or at risk of non-self-sufficiency, and people with serious disabilities
2. “Paths for the care and family support of people suffering from dementia” which provides for the financing of assistance services for people with dementia residing in Tuscany
3. “Expansion of the home care service” which provides for the provision of a monthly contribution for the family assistant (regularly employed with a contract of at least 26 hours per week) with the aim of reducing the costs borne by the patient and the families involved and therefore encourage home care and assistance for as long as possible
4. “Home services of a social-welfare nature for minors with disabilities” which provides for the financing of social-welfare and socio-educational services for minors with certified disabilities (Law 104/1992) residing in Tuscany
