CATANIA – They were carried out in the operating rooms of the Arnas Garibaldi in Catania the first bariatric surgery operations with full robotic technique. The patients are seven women aged between 20 and 46, with a body mass index greater than 40. They present with sleep apnea syndrome, hypertension and diabetes.
They were subjected to sleeve gastrectomy and gastric bypass. The post-operative course was uneventful. The patients began drinking again a few hours after the operation and moved independently after just twelve hours. The requirement for analgesics was minimal.
The post-operative course
On the third postoperative day the patients they returned to their home with maximum satisfaction. “When we offered our patients the possibility of being operated on with the aid of robotic technology – explains professor Luigi Piazza – each of them accepted with conviction”.
Piazza is the director of the complex general and emergency surgery unit, as well as director of the surgery department. “We explained how the operation would take place and the advantages that this technique offers. The patients were there documented and welcomed the opportunity with great confidence”, continues Piazza.
The technique used
This type of surgery, underlines the professor, intervenes on patients who have, by definition, “of size and above all a quantity of adipose tissuein the abdomen such that the vision of the operating field is often not optimal and therefore has a negative impact on the precision of the surgical procedure”.
“The 3D stereo vision system with which the robot is equipped, however, together with the instrumentation that amplifies the movements of the surgeon’s hand, make it possible to overcome these limitations – he underlines -. In fact, everything offers a extraordinarily defined vision and precision of the act achieved more easily.”
“The results obtained in terms of precision and quality of the intervention are comparable to those of traditional laparoscopic techniquebut the ease with which they are obtained really makes the difference, especially for the more complex cases.”
Robotic surgery
“Initially reserved for some specialist branches such as prostatic, gynecological or hepatobiliopancreatic surgery, in recent times the Robotic surgery has also been extended to other surgical fields”.
“Bariatric surgery falls into one of these new fields of application albeit still with an initial and growing experience. In Italy today there are few cases of bariatric surgery that are carried out with the robotic procedure”.
“Our bariatric center of excellence has been recognized by SICOB (Italian Society for the treatment of obesity and metabolic diseases) for over 20 years, after years of learning and equipped with the best technologiesaims to become a reference center also in the field of robotic bariatric surgery”.
The next interventions
Other interventions are already planned for the next few weeks. “Our goal – she concludes – is to continue negotiating the majority of bariatric patients in the operating rooms of Garibaldi Arnaswith the traditional laparoscopic technique, and intervene with the support of the robot on the most complex cases such as higher BMIs or revision bariatric surgery”.
General Surgery at Arnas Garibaldi is central Regional HUB for bariatric surgery. It takes care of bariatric complications and failures in patients who have undergone surgery at other centers as well. And it does this through revision surgery, also called “redo-Surgery”.
The Arnas Garibaldi Obesity Surgery Center is also a reference in the specialist bariatric surgery training network national and international, being a training center for the ACOI-Sicob “U. Parini”, “and of the international school of the MGB/OAGB Academy.
To complete the obesity surgery course, at the General Surgery Unit directed by Prof. Luigi Piazza, the post-bariatric reconstructive plastic surgery servicewhich deals with the body remodeling of formerly obese patients in order to resolve the excess skin that has arisen following weight loss.
