On Tuesday 16 April 2024, the General Directorate for Pharmaceutical Welfare and Medical Devices of the Lombardy Region published a note, signed by the manager Ida Fortino, with various operational instructions. The document specified that «even for titrated extracts it is advisable for the doctor to draw up a prescription indicating the quantity of titrated extract in volume, since these extracts are supplied to the pharmacy in liquid form and loaded into the entry-exit register of narcotics in ml (milliliters)”.

Exemption for quantities multiples of 50 ml
Following this note, the general management published on April 29th a further clarification entitled “Titrated cannabis-based extracts – integration” in which it specified that, in addition to the note of April 16th, «the validity of the plans is confirmed therapeutic products issued before 16 April 2024″. Furthermore «the prescribed quantity must be 50 ml or multiples thereof. The doctor may derogate if he believes that multiple quantities of 50 ml could lead to problems relating to patient safety. In this case the doctor will be able to prescribe an appropriate number of milliliters in relation to the dosage reported in the therapeutic plan and in the prescription”. Finally, “at least 1:1 dilution is required in the presence of titrated extracts with a THC content equal to or greater than 5%”.
