Managing the disease in an integrated way: this is why the “clinical-care networks” were created. The Marche Region has defined the guidelines, a fundamental step, which implements the strategies outlined in the regional health and social plan. The clinical networks represent “functional aggregations” of services and operators who work together in a continuous and coordinated way to achieve common objectives and guarantee high quality care pathways, as envisaged by Ministerial Decree n.70/2015 and implemented by the Region’s Social and Health Plan Marche 2023-2025.

The objective is to make the healthcare offer in the Marche more accessible, orienting citizens and healthcare workers for global management. The networks are divided into time-dependent and non-time-dependent. The time-dependent ones are those in which the time factor is decisive and are: cardiology for emergencies, stroke, trauma, neonatology and birth points.

The model identified is the “Hub and Spoke” model where the Hubs are responsible for managing the most complex cases, with highly specialized staff, advanced technologies and resources to manage the high intensity of care. The Spokes, on the other hand, provide less complex services and guarantee continuity of care by collaborating with the reference Hub.

Non-time-dependent networks instead focus on an integrated management of the disease that goes from prevention, to diagnosis, to treatment, to rehabilitation. For both, governance is based on two levels: a Regional Coordination and Local Committees at the Ssr Bodies. Regional Coordination involves the presence of the director and competent managers of the ARS, the medical directors of the SSR entities participating in the network and a Clinical Contact person as operational manager of the network.

Among the main activities: the promotion of Diagnostic-Therapeutic-Assistive Paths (PDTA), the development of protocols and procedures for taking charge and satisfying the needs of citizens, clinical assistance and organizational audit activities, verification of activities and control of objectives.

The regional clinical networks, through the attribution of specific organizational responsibilities, aim to guarantee the most adequate skills and the most effective responses, equity of access to services based on need, but also adequate use of personnel, technology and spaces. The final objective is to overcome the corporate or structural logic to respond in a broad and coordinated way to complex problems, in implementation of the State-Regions Agreement of 24 January 2018.
