A new pathogenetic model and treatment principles

Auxologico organizes a in-person event dedicated to the topic of lumbosciatic pain, on the occasion of the release of the book “Lumbosciatic pain: a new pathogenetic model and treatment principles” by Prof. Luigi Tesio, published by Il Pensiero Scientifico Editore. In this text, Prof. Tesio presents the CoVIn pathogenetic model for lumbosciatica, i.e. the very widespread “backache”.

The lower back and sciatic pain it may seem like a minor issue, yet sooner or later it affects over 80% of the population, and for this reason it is the condition that generates the greatest need for rehabilitation (as reported by the WHO). Nonetheless, it is still a poorly defined clinical condition (as the name itself reveals), which therefore lends itself to various and contradictory therapeutic approaches. The reason is that there is not yet a clear etio-pathogenetic model that explains why and how the problem arises.

The dominant pathogenetic models for low back and sciatic pain they are compressive and inflammatory, which however do not justify many contradictory observations: the poor correlation between pain and diagnostic images, pain that appears at rest and not under load, pain during pregnancy, spontaneous healing or in response to very different treatments , and others.
L’missing link, according to Prof. Luigi Tesio, is the role of the epidural venous plexus, the fourth bodily venous system, discovered in 1940, which internally lines the rigid spinal canal. The plexus is a-valvular and therefore can dilate to transmit any increase in venous pressure of extravertebral origin or following local compression. This causes pain and possible chronicity due to phlebitic-fibrotic evolution. Venous stasis, therefore, becomes the cornerstone of a new pathogenetic model called CoVIn (compressive-venous-inflammatory).

Many contradictions can be explained through intracanal venous dilation (for example, pain at rest and not under load) and thus opens the way to a rational and simple approach to conservative treatment: spinal flexion exercises, water exercise and active lumbar traction. Surgery is therefore limited to extreme cases. The CoVIn model obviously does not explain every condition of lumbosciatic pain but it is a valid proposal for a pathogenetic model for most of the most common forms of pain: an opportunity for progress in an area of ​​medicine in need of much more clarity than usual. currently have.

“Back pain”, in an era in which gene therapies and stem cell treatments are appearing, risks remaining hidden behind the appearance of a minor pathology. In reality, the problem affects many people. The author proposes a new mechanism that explains why and how lumbosciatica pain arises. This new model explains the diversity of the clinical pictures observed and proposes a rational approach to the choice of therapies.

The book represents a sort of programmatic “manifesto” that will inspire a specific, clinical and scientific collaboration between Auxologico and the Policlinico di Milano Ospedale Maggiore Fondazione IRCCS Ca’ Granda.
