The project is part of the regional strategies of the Piedmont for the management of the chronicity. Meanwhile, represents an operational tool to translate the clinical guidelines in concrete practices, adapted to the local context.
Everything is the result of the work of a multi -professional group which involved specialists. general practitioners, nurses, pharmacists, social workers, psychologists and dieticians. After a careful analysis of thehealth organization already existing. the group has divided into three thematic subgroupseach dedicated to a specific asl to5: new path chronic part of the path.
The result is a shared document which defines clearly roles. actions and times assistance to the patient with the affected by chronic kidney diseaseapproved with resolution of the General Manager of the ASL TO5.
